Special and vulnerable populations (SVP) often face additional barriers to care, many of which are compounded by social determinants. When screening for Social Determinants of Health (SDOH), health centers serving SVP will need to take into account the unique needs and circumstances of the populations they serve, particularly during times of crisis (e.g., COVID-19 pandemic). Screening for SDOH is the first step towards addressing these disparities.
