The Health of Asian American and Native Hawaiian/Pacific Islander Patients Served at Health Centers: An Analysis of the 2021 Uniform Data System
Association of Asian Pacific Community Health Organizations (AAPCHO) hosted a webinar sharing a data analysis of the 2021 Uniform Data System (UDS) and recommendations to increase access to care and improve health outcomes for AA and NH/PI patients. Presenters shared key findings from AAPCHO’s updated and annual report, “The Health of Asian Americans, Native Hawaiians, and Pacific Islanders Served at Health Centers: An Analysis of the 2021 Uniform Data System”.
Social Determinants of Health Lessons Learned, Challenges, and Barriers: A Resource for Health Centers Vol. 3
Special and vulnerable populations (SVP) often face additional barriers to care, many of which are compounded by social determinants. When screening for Social Determinants of Health (SDOH), health centers serving SVP will need to take into account the unique needs and circumstances of the populations they serve, particularly during times of crisis (e.g., COVID-19 pandemic). Screening for SDOH is the first step towards addressing these disparities.
